CLS Student Application for Admission Download

Directions for completion of application (This application can also be used for returning students. All documentation must be updated yearly.)

1. Download application (You must download the application in order to fill it out. This process cannot be done on a phone.)

2. Fill out application completely (Once you fill out the application click save as and save to your computer.)

3. Click “Submit” at the end of application. (If you are having problems submitting, create an email to fmacon@concordianola.com or mdeidrich@concordianola.com. In the subject line put your child’s name, go to the bottom of the page and look for the attachment or paperclip, click on it to find the application you save, if you are having problems finding it type the date in the search bar find the file, attach it to the email and hit send.)

APPLICATION IS INCOMPLETE UNTIL APPLICATION FEE IS RECEIVED THROUGH GULF COAST BANK & TRUST PAYMENT PORTAL.
